In this activity, an expert provider, navigator, and two patients explore the factors that contribute to people living with HIV (PLWH) falling out of care and different strategies that can be used for re-engaging PLWH back into the HIV care continuum. Upon completion of this activity, participants should be better able to: Identify individual and structural factors that contribute to PLWH falling out of care; Discuss methodologies, benefits, and limitations associated with different strategies for re-engaging PLWH who are out of care; and Employ individualized and comprehensive plans to successfully re-engage PLWH in care
